Alternatively, you may choose to print it and return via email or in person. If you prefer to do it on the day of your appointment, please arrive 10 minutes early to allow time. All patients require a current referral prior to their appointment.

All patients with prolapse or urinary symptoms are asked to complete the Australian Pelvic Floor Questionnaire before their first appointment.

Those patients with urinary symptoms will also be asked to complete a bladder diary and to bring it to their first appointment.
